Most of the interviews for this book were conducted between September and December 2010. A few took place in March and April 2011. The majority were in person, sometimes followed up by a phone call or e-mail. The locations ranged from people's homes and offices to coffee shops and my office. Several interviews had to be done by phone for logistical reasons, and one was done by e-mail, at the person's request. Two individuals asked to see a draft of their comments, prepared for publication. This was provided, and both persons approved.
